MetLife Medical Plan – Updated Booklet & Registration Information

We are delighted to announce that the updated Medical Booklet is now available. Please find it attached for your reference.

For those interested in enrolling in the MetLife Medical Plan, please note the monthly contribution rates:

• Employee only: €23.20 per month
• Employee + 1 dependent: €42.40 per month
• Employee + 2 or more dependents: €67.60 per month

To proceed with your enrolment, kindly complete the Group Insurance Enrolment Form, ensuring all three pages are fully completed and signed.

Once completed, please return the form to me.
